Check-List.

If you would like to submit an application for support in the field of music to ART MENTOR FOUNDATION LUCERNE, please use the following check-list. It defines the structure and contents of your application.

1. Project Description
	Summary of project on 1 or 2 A4-pages (or 8 ½ x 11 inches).
	Short description of institutions and/or performers and artists involved.
	Reason why the application corresponds to the purpose of the Foundation and the conditions of ART MENTOR FOUNDATION LUCERNE.
	Information regarding sponsorship and organizer.
	Data, deadlines and time schedule.
	Location of event.
	When possible and appropriate, please include test recording (e.g. a CD).

2. Information on persons responsible for the Project
	in each case:
	Name and address
	Profession and function
	Previous experience with similar projects
	List of references

3. Budget and Financing
	Budgeted revenue and expenses
	Level of self-financing
	Amount of support requested
	Amount of support already committed

4. Other Sponsors and Institutions already involved
	in each case:
	Description, contact person and address: grants already promised sponsor awards
	Description, contact person and address: amounts already requested
